Privacy Practices

Community Memorial Hospital is required by law to protect the privacy of your health information and to provide you with a copy of our Notice of Privacy Practices (PDF). This Notice describes the ways we will use and disclose your health information. This Notice also describes your rights and our obligations regarding the use and disclosure of your health information.

No Visitor PolicyEFFECTIVE IMMEDIATELY

Community Memorial is putting in place a ‘no visitor’ policy as advised by the NYS Department of Health, effective immediately.

This applies to all patient care areas except the following.

Patients in the Comfort Care area are allowed one visitor only.

 As a reminder:

• Visitors with respiratory symptoms, fever, cough or other flu-like symptoms are not allowed.
• Children under the age of 15 are not allowed.
