Visitor Policy Post Pandemic

Under NYS guidelines outlined by Gov. Cuomo, visitors will be allowed to return to Community Memorial Hospital under strict protocols outlined by the Hospital to protect all that enter our building and our Health Centers.

As a result, it is necessary for our Hospital and Health Centers to institute precautionary measures to ensure that our institutions are safe for our patients, their families, and friends, as well as our healthcare workers. For this reason, we will take the following measures on a temporary basis that may change at any time due to Covid-19 outbreaks:

For Community Memorial Hospital:

Visiting Hours will be strictly enforced and are limited to admitted patients only. At this time the OR and ED will continue with the NO visitor policy. Exceptions may be allowed on a case by case basis with leadership approval.
 
Visiting Hours will be 4p-8pm ONLY
 
Restriction on visitors:

  • Each patient will only be allowed 1 registered visitor at a time for a maximum of four hours per day. Only two visitors, in total, are permitted during the four hours of visitation per day.
  • Visitors must maintain social distancing of six feet, even in patients rooms.
  • Children under the age of 18 will not be allowed to visit.
  • Visitors are not permitted to visit any patient who is in isolation, unless exceptional circumstances and prior approval have been obtained.
  • Visitors are to remain in the patient room at all times unless directed by staff to leave due to a treatment.

 
Exception: *Exceptions will be made for patients with special needs, pediatric patients and patients in immediate end of life situations.
 
Visitor entrance and exit:

  • Visitors must use the designated door to enter and exit the building for visiting at the Registration/ER entrance to the hospital.

 
Visitor Screening:

  • All visitors entering the building will be asked the REQUIRED screening questions before being allowed to visit and shall be denied entry if they answer yes to any of the questions related to COVID-19 exposure or had or have symptoms during the prior 14 days prior to visiting or have a temperature equal to or greater than 100.0 degrees Fahrenheit.
  • All visitors will be provided education on wearing a mask and hand washing. Those who answer yes to any of the screening questions will be asked to follow up with their Primary Care Physician or seek care in the Emergency Department.
  • Visitors who fail to wear a mask or refuse to wear proper PPE will be asked to leave the facility.

 
Visitor Registration:

  • You will be required to sign a visitor registration log designated to track patient visits in the hospital according to Governor Cuomo’s mandate for the purposes of infection control.
  • A visitor pass will be provided and should be returned at the entrance following the visit.
  • All visitors will also be provided with a list of guidelines that include information on hand hygiene, wearing a mask, practicing social distancing and contact

 
Visitor Responsibilities:

  • You are asked to stay in the patient room. Please be advised against moving around to other patients and sitting on patient’s beds.
  • You must wash/sanitize your hands upon entry to the hospital, before and after any contact with the patient, and prior to leaving the hospital.

For Community Memorial Family Health Centers, Specialties and Urgent Care:

Restriction on visitors:

  • Each patient will only be allowed 1 registered visitor or parent for pediatric patients.
  • Visitors must maintain social distancing of six feet at all times.
  • Children under the age of 18 will not be allowed as a registered visitor.
  • Due to limited waiting room capacity and the need for social distancing at our sites, a visitor may be asked to wait outside until patient they are with is called back to an exam room.
  • Visitors are to remain in the exam room at all times unless directed by staff.

 
Visitor Screening and Registration:

  • All visitors entering the center will be asked the REQUIRED screening questions before being allowed to accompany the patient and shall be denied entry if they answer yes to any of the questions related to COVID-19 exposure or had or have symptoms during the prior 14 days prior to visiting or have a temperature equal to or greater than 100.0 degrees Fahrenheit.
  • All visitors will be provided education on wearing a mask and hand washing. Those who answer yes to any of the screening questions will be asked to follow up with their Primary Care Physician or seek care in the Emergency Department.
  • Visitors who fail to wear a mask or refuse to wear proper PPE will be asked to leave the center.
  • You will be required to sign a visitor registration log designated to track patient visits in the center according to Governor Cuomo’s mandate for the purposes of infection control. If waiting is at capacity in regards to social distancing, visitors will be asked to wait outside until patient is taken to an exam room.
  • Signage is posted in the centers providing the guidelines that include information on hand hygiene, wearing a mask, practicing social distancing and contact. There is front office staff to answer additional questions, should they arise.

 
Visitor Responsibilities:

  • You are asked to stay in the exam room at all times unless directed otherwise.
  • You must wash/sanitize your hands upon entry to the center, before and after any contact with the patient, and prior to leaving center. Sanitizer is available throughout the centers for ease of access.

  
All visitors will also be provided with a page of guidelines that include information on hand hygiene, wearing a mask, practicing social distancing and contact.
