Social Services

The Social Services Department at Community Memorial Hospital provides discharge planning for its patients to ensure home safety and continuity of care once the patient leaves the hospital. Services provided by the department include but are not limited to:

• Public Health referrals
• Short term rehabilitation referrals
• Referral for long term placement in a nursing home
• Outreach services which include information for Lifeline,
  home delivered meals, Home Helper list
• Durable Medical Equipment for home use
• Health Care Proxy forms
